What to expect at your first appointment

​

Accredited Practising Dietitians (APDs) are Australia's most trusted expects in nutrition. They understand how nutrition affects the body and will give you expert nutrition and dietary advice. APDs provide personal support to help you improve your health and wellbeing. 

​

Supple Clinic Dietitian have experience in treating a wide range of chronic conditions, gut diseases, eating disorders and hormonal issues. Our dietitian is also expert in delivering advice in a no-nonsense and easy-to-understand way, so that you feel empowered to make sustainable healthy habit changes. 
 

Our Dietitian will take a detailed history of your concerns, run through your food intake, lifestyle, job, sports/ hobbies, supplements, medications and medical history. They will then perform a detailed assessment, explain your contributing factors to you in plain English, treat you using evidenced-based advice, and set you up with a personal action plan.

When making your appointment

​

Choose your appointment type - online or phone. We currently are not offering in clinic or home visits. 

​

Please forward any referral letter or Medicare or DVA Plan to us PRIOR to your appointment, otherwise we may not be able to apply the Medicare/DVA plan to your appointment, and you will not get the rebate. 

​

A referral is NOT a requirement for an appointment. 

​

If you have a Return to Work SA claim or an Insurance claim - please get your Claims Manager in touch with us to make arrangements PRIOR to your appointment. Otherwise, if urgent, you can come to your appointment, you pay upfront, and organise the rebate from your Claims Manager yourself (we will give you a receipt). 

​

If you have an NDIS Plan - again we require the details prior to your first appointment.

​

Upon booking your appointment, you will receive an email with a secure link for you to complete forms online before we see you. If you do not have time before hand to fill in our Intake Form, please arrive 10 minutes early to complete paperwork on the day.

​

If you have questions, you can book into a Free Nutrition Screening phone call. This is a 10 minute chat with our Dietitian where you can summarise your concern and find out if we can help you.

What to bring to your appointment

​

Please bring the following:

  • Recent test results

  • Contact details of any health professional you wish for us to communicate with on your behalf.

  • Your Medicare, Private Health Fund or DVA card (whichever relevant) to ensure accurate rebate or on-the-spot claiming.

  • Any letter from your GP or Specialist - if you haven't given to us already.

In addition you may find it useful to complete one of these and bring to your appointment:

  • Food & Exercise Diary

  • Food, Hunger & Mood Diary

  • Gastrointestinal Symptom Questionnaire

​

  • We welcome you to bring a family member or friend as support to your appointment. All children (under 18 years) must be accompanied by an adult for the whole appointment.
